Understanding PRK vs. LASIK 

While LASIK is a highly recognized term associated with laser vision correction, many patients are unaware of the common alternative, photorefractive keratectomy (PRK). When comparing PRK and LASIK, it is important to know that both provide patients with clearer vision. However, PRK offers a safer solution both during the procedure and during your recovery afterward. Where PRK corrects vision by removing part of the outermost layer of the cornea, LASIK creates a flap and eliminates tissue from beneath.

Procedural Differences

When performing LASIK, it is necessary to create a flap before reshaping the cornea to access the underlying tissue. The flap is then closed and typically heals on its own without the help of sutures.while the edges seal overnight the interface never heals fully, leaving the patient vulnerable forever to flap dislocation from severe rubbing or even a mild poke in the eye. However, it has been proven that the inner layers of the flap never fully heal, leaving the patient vulnerable forever to flap dislocation from extreme rubbing or being poked in the eye.

However, with PRK, the doctor can effectively alter the cornea without creating a flap. Instead, the surgeon will gently remove the outermost layer of the cornea using alcohol instead of cutting into the tissue. This exceptionally thin layer will grow back during the healing process within just a few days.

Who Qualifies for LASIK and PRK?

Generally, patients who are candidates for LASIK will also be suited for PRK, as well. However, without the need to create a flap, PRK also allows patients with thin corneas to receive laser vision correction. Consequently, candidates who do not qualify for LASIK can usually opt for PRK instead.

Safety Considerations

While LASIK is a reputable procedure, the PRK surgical technique provides patients with several distinct safety advantages, including: 

No Flap-Related Complications

By performing the entire procedure on the surface of the cornea, PRK completely eliminates the risk of flap-related complications. Potential issues that can arise from flaps include:

  • Wrinkling
  • Displacement
  • Trapping of debris
  • Inflammation

PRK not only sidesteps these complications but also ensures that more of the corneal tissue remains intact, allowing the tissue to remain stronger. However, patients should keep in mind that the procedure still carries a risk of post-surgical infection. 

Future Procedures

In rare cases, patients who have undergone laser vision correction need future retreatments due to changes in their eyesight. Patients who have undergone PRK will have a greater amount of corneal tissue to work with. For patients who have undergone LASIK in the past, Dr. Gold typically recommends PRK for retreatment, since lifting an older corneal flap can lead to epithelial ingrowth and risk serious complications. 

Recovery & Results

Because no corneal cutting occurs, PRK potentially provides better visual results than LASIK, allowing patients to enjoy significantly improved vision and reducing or eliminating reliance on corrective eyewear. However, PRK does require a longer recovery period.
